Labyrinth of Madness is a 64-page adventure module for Advanced Dungeons & Dragons 2nd edition released in August 1995. It is for a party of at least 15th level, and ideally higher.

Sinister, twisting images... Horrific nightmares lurking at the corners of the mind... These are descriptions used to tell the tales of the Labyrinth of Madness. But these tales of the labyrinth are only legends, really, nothing more than stories used to frighten children at night–until a mysterious scepter is found, bearing within its crystal head a visage of insanity and terror, and also delivering a message: "Disturb not the Labyrinth of Madness again, and live a while longer." Now a powerful temple suffers from a tragic curse that is somehow linked to the labyrinth. Does there exist a group of heroes who can penetrate this dark and terrible place to life the curse... and survive? Labyrinth of Madness is a multiple-level, three-dimensional dungeon adventure, a puzzle within a puzzle, that commemorates 20 years of gaming with TSR.

Backstory[]
Over 800 years ago, an evil species of humans called the dyris fled to the Underdark, where they could freely practice their culture of human sacrifice and torture. There they opened gates to Pandemonium and the Abyss, carelessly invoking a semi-sentient force of chaos called the lifebane. The dyris were corrupted into serpent-like species, becoming yuan-ti abominations. Finally, the dyris priest Im-Ravin made himself a lich, burying himself and his people within a tomb called the Labyrinth of Madness.

The book was written by Monte Cook. Art was provided by Jeff Easley and Dan Frazier.
The number 20 appears various times, as this product celebrates the 20th anniversary of TSR.

Labyrinth of Madness was released by TSR in August 1995 for $9.95 US, $11.95 Canadian, or £5.99 UK.
On August 6, 2013, it was re-released in digital format. It is currently available on DriveThruRPG and Dungeon Masters Guild for $4.99.
